The difference in those Fedora iso and are the packages that include, for instance Fedora Server is a setup to be use in a server for that reason it do not include any graphical desktop and include a tool to manage the server via web browser and other server tools. Fedora Workstation include desktop tools like Libre Office and use Gnome Desktop. Fedora Spins include a different desktop like KDE, LXDE, Xfce, Cinnamon and desktop tools Fedora Design include all design Free tools like Gimp, Inkscape, etc.

Not that you cannot include those tools on a Fedora installation, but depending of what you do, it may take a bit of time and knowledge to install those packages. Normally it will when you do that on Fedora Server since it do not have graphic desktop.

So basic difference are the packages that include. No more no less.

Hope this help
